Well Child Check-up, Immunization, and Screening Schedule

Newborn None if the first Hepatitis B was given in the hospital.
2 Weeks None. The mandatory State Newborn Metabolic Screening Test and the optional Baylor Metabolic Screening Test are performed.
2 Months Pentacel (DTaP/ IPV/Hib), Prevnar (Pneumococcal Conjugate), RotaTeq (Rotavirus), Hep B
4 Months Pentacel (DTaP/ IPV/Hib), Prevnar (Pneumococcal Conjugate), RotaTeq (Rotavirus)
6 Months Pentacel (DTaP/ IPV/Hib), Prevnar (Pneumococcal Conjugate), RotaTeq (Rotavirus) , Hep B
9 Months None, Developmental screening questionnaire, Anemia screening
12 Months MMR (Measles/Mumps/Rubella), Varicella (Chickenpox), Hep A (Hepatitis A)
15 Months Pentacel (DTaP, IPV, Hib), Prevnar (Pneumococcal Conjugate)
15 Months Anemia screening
18 Months Hep A, Autism and developmental screening questionnaire
2 Years None, Autism screening questionnaire
30 Months Developmental screening questionnaire
3 Years Well check yearly from 3-18 years
4 Years Quadracel (DTaP/IPV), Proquad (MMR & Varicella)
5-10 years Annual preventative well child check-ups are as important for children as they are for adults even when a child is healthy and immunizations are up-to-date. 9 year well check has lipid screening.
11-12 Years Tdap (Tetanus-Diptheria-Pertussis), Menactra (Meningococcal), Gardasil (Human Papilloma Virus)
16 Years Menactra (Meningococcal) booster
18 Year Bexsero (Men B), prior to college
